Work Study Forms
First-time FWS (students who have never been paid through WCCCD's payroll system before) are required to complete and submit the forms listed below.
- FWS Agreement
- City Tax Form (if student is working or living in the city)
- Federal W-4 Tax Form
- MI W-4 Tax Form
- Immigration and Naturalization Services (INS) I-9 Form
- Earnings and Allowance Form
- Confidentiality Agreement Form
- Direct Deposit Form (Please submit w/voided check to Human Resources Department)
Once these forms are completed, the campus president's office will complete a requisition form to officially place an employee on the Payroll system.
Any address changes must be made with the Campuses Student Services Departments. The I-9 form is required by federal law before anyone is hired in a new job. It proves identity and employment eligibility.
Returning FWS Students
Returning FWS students returning to the same position must attend the mandatory orientations held annually (every Fall semester). Once a student begins his or her assignment within the FWS program, they cannot transfer to another location until the next semester.
The forms required of FWS student employees by the Federal Work Study Office are listed below. Blank forms can either be downloaded from this page, or can be obtained directly from the Federal Work Study Office. PLEASE NOTE: WCCCD students without a Federal Work-Study award listed on their Financial Aid Award Notice may NOT use these forms.
