Work Study Forms
First-time FWS (students who have never been paid through WCCCD's payroll system before) are required to complete and submit the forms listed below.
- FWS Agreement
- City Tax Form (if student is working or living in the city)
- Federal W-4 Tax Form
- MI W-4 Tax Form
- Immigration and Naturalization Services (INS) I-9 Form
- Earnings and Allowance Form
- Confidentiality Agreement Form
- Direct Deposit Form (Please submit w/voided check to Human Resources Department)
Once these forms are completed, the campus president's office will complete a requisition form to officially place an employee on the Payroll system.
Any address changes must be made with the Campuses Student Services Departments. The I-9 form is required by federal law before anyone is hired in a new job. It proves identity and employment eligibility.
Returning FWS Students
Returning FWS students returning to the same position must attend the mandatory orientations held annually (every Fall semester). Once a student begins his or her assignment within the FWS program, they cannot transfer to another location until the next semester.
The forms required of FWS student employees by the Federal Work Study Office are listed below. Blank forms can either be downloaded from this page, or can be obtained directly from the Federal Work Study Office. (You will need Adobe Acrobat Reader in order to view and print the following documents. This free software is available for download from the Adobe website at www.adobe.com.)
PLEASE NOTE: WCCCD students without a Federal Work-Study award listed on their Financial Aid Award Notice may NOT use these forms.
Form Information
Although the FWS Program receives funds from the Federal Government, income earned through the FWS program is not exempt from city, federal or state taxation. Therefore, each FWS student must complete city, state and federal tax withholding (W-4) forms.
If you do not live or work in the city of Detroit, please complete the top of the document by writing that information at the top with your demographic information in the provided section and sign the bottom of the form.
Any questions regarding tax forms should be directed to District Financial Aid and Human Resource Department 313-496-2765.
FWS student employees who handle private or sensitive information in the performance of their jobs must be informed of the District’s policy on confidentiality, and will be asked to sign a confidentiality agreement. The signed agreement must be kept in the supervisor's records.
Immediately upon being hired, the supervisor, FWS student, Vice-Chancellor or Campus President must fill out a Federal Work-Study Authorization Form, authorization forms are provided upon paperwork completion. The Authorization Form serves several purposes:
1) It is proof to the supervisor that the student is an authorized participant in the FWS Program.
2) It informs both parties of the student's earnings limit.
3) It is a record maintained in the student's file showing where he or she has been employed.
You will not be able to work until the documentation and authorization form is received by the Campus President's Office and submitted to the Financial Aid Department for processing.
